What to Expect from Ear Surgery

Frequent sun exposure is unfortunately one of the most common contributing factors to wrinkles and photoaging. The sun also causes sunburn and increases your risk of skin cancer. For those reasons, most doctors and experts agree that using sunscreen is vitally important, and not just while you're at the beach, but anytime you are out in the sun for more than a few minutes at a time.
Enter the wide array of sunscreen products. According to a new study, a good portion of people do not understand all of the information displayed on sunscreen labels, which can make it difficult to know which one is the best one to buy for your purposes and skin type.
The study was conducted with 114 participants at the Northwestern Medicine dermatology clinic in the summer of 2014. The participants were asked about how certain sunscreens were related to sunburns, aging and skin cancer based on the information on the labels. Here are some of the highlights of the findings:
- 80 percent of those surveyed said they had purchased sunscreen in 2013.
- 75 percent said prevention of sunburn was the number 1 reason they bought sunscreen, followed by prevention of skin cancer.
- Only 43 percent of those surveyed could define what "sun protection factor," or SPF, means.
- The top 3 reasons respondents decided to buy one sunscreen over another was high SPF, sensitive skin formula, and sweat and water resistance.
- About half of respondents bought sunscreen with the highest possible SPF.
The highest SPF won't necessarily benefit you. SPF gives you information about sunburn prevention, not photoaging and skin damage. SPF rates the degree of solar intensity that would be needed to produce a sunburn. An SPF of 100 does not mean you are 100 percent protected; the only way to be 100 percent protected is to stay out of the sun.
Most participants knew that SPF refers to sunburn protection, but only 33 people knew SPF does not tell you anything about protection against UV-A rays. UV-A rays are associated with aging your skin, while UV-B rays are associated with sunburn. Both UV-A and UV-B can contribute to skin cancer. For that reason, it's important to choose a "broad spectrum protection" sunscreen, which means it blocks out both types of radiation.
So what sunscreen should you choose? According to this study, effective sunscreen use includes the following aspects:
- Labeled as offering "broad spectrum protection"
- SPF of at least 30
- Use about an ounce of sunscreen each time you apply (equivalent to the volume of a shot glass)
- Reapply every two hours

A new study published in the Aesthetic Surgery Journal says that use of contact lenses contributes to upper eyelid droopiness. These findings are based on a study comparing pairs of identical twins. The twins who wore contact lenses showed more sagginess of the upper eyelids.
Does that mean you should stop wearing contact lenses? Not necessarily. Contact lenses provide freedom from what can be the hassle of wearing glasses. Many people find contact lenses allow them to be more active, and some people really prefer the way they look without glasses. It's a personal preference. If you're really worried about how lenses could affect your eyelids, discuss the issue with your eye doctor.
The amount of drooping caused by contact lenses is likely minimal. The sagging can result from the eyelid being stretched each time you place and remove your lenses. The study found the sagging was more pronounced in people who used hard contact lenses.

After breast augmentation, you need to take some time to rest and let your body heal. Take the opportunity to let yourself relax and take a break from your normal responsibilities, chores and daily routine. Each woman will experience a slightly unique recovery process, but below we've provided a general timeline of what you can expect after breast augmentation.
One Week after Breast Augmentation
For the first week, it's completely normal to feel stiff and sore in your chest region. Your breasts will initially be bandaged with surgical dressings, which experienced plastic surgeon Dr. Joel Beck will remove within the first week. Lifting and strenuous upper body activity should be avoided during this time. Dr. Beck will let you know when you can return to work, which is generally within a week.
Two to Six Weeks after Breast Augmentation
During the next few weeks, your breasts will continue to heal. You'll notice a significant reduction in swelling and bruising, but your breasts will probably still be tender. Certain physical activity and types of exercise will be restricted as instructed by Dr. Beck.
Your Long-Term Recovery and Results
Although your finalized breast results should be apparent after about 6 months, a very minor amount of swelling will continue to subside for up to a year. The results of breast augmentation are long-lasting, and although your breast implants may need to be replaced at some point in your lifetime, you should continue to have beautiful, natural results for a long time to come.

The American Society of Plastic Surgeons says rates of body and skin surgeries, often performed for patients who lost a lot of weight, grew at the fastest rate in four years in 2014. ASPS says these post-weight loss procedures experiencing growth include:

After a breast enlargement procedure, you will need to take some time off from work, intensive exercise, and other specific activities as directed by our board-certified plastic surgeon Dr. Joel Beck. During your consultation and in the appointments leading up to your surgical day, we will make sure you have all the aftercare instructions and information you need to recover successfully. Your recovery may differ slightly from the general outline below:

At Bay Area Aesthetic Surgery, experienced plastic surgeon Dr. Joel Beck has had a few women ask him whether they can combine a Brazilian Butt Lift, tummy tuck and breast augmentation all into a single surgical appointment. Other cosmetic surgeons in Northern California had offered this option to these women. So, is it safe or even desirable to combine fat transfer to the butt with these other procedures? Dr. Beck's conclusion is that it is not safe to do so for the following reasons:
- Incompatible recoveries: After fat transfer to the buttocks, you will be unable to sit for at least 2 to 3 weeks. But after a tummy tuck, you will have to sit and lie down on your back to recuperate. These opposing recovery demands are often overlooked by surgeons who offer to combine these procedures.
- Risks are too great: Undergoing all of these procedures at once is unsafe. The greatest risk posed is that of an increased incidence of blood clots. There would also be an increased risk of clots travelling to the lungs or heart, both of which are life-threatening situations. The risk goes up particularly when tummy tuck is combined with liposuction. There would also be an increased risk of infection. Fat transfer to the buttocks could cause an infection that could affect the results of the breast augmentation, tummy tuck or the butt lift.
- Risks outweighing the benefits: Although you'd only need to go under with anesthesia once and you could save financially, these benefits are outweighed by the heightened risk of complications and poor cosmetic results.

No, breast implants do not always harden over time. Instead, this hardening of the implants is a condition known as "capsular contracture." This condition is a complication that is possible after any breast augmentation surgery, but it's not very common. And if it should arise, there are procedures for treating and reversing it.
Capsular contracture occurs when the natural scar tissue surrounding your breast implants tightens and hardens, resulting in a breast that looks distorted and feels hard. It can occur in one breast or both breasts, and is rated from grade 1 to grade 4 depending on how bad it is. In the most advanced grades of capsular contracture, your breasts can feel painful in addition to feeling hard. To reverse the complication, a surgical procedure can be performed to correct the issue. In a small number of advanced cases, the breast implants themselves may need to be removed and replaced.
Sometimes capsular contracture is unavoidable, but strictly following pre-operative and post-operative guidelines from your plastic surgeon can reduce the risk of this complication.

Researching breast augmentation involves coming across a lot of surgical terms. Here are definitions of some of the more common ones:
Breast implants: medical devices placed inside your breasts to increase breast size and improve their overall shape. Implants have an outer silicone shell, and saline solution or silicone gel fill material.
Periareolar incison: an incision made around the perimeter of the areola, through which the breast implants are placed in the breast pocket.
Inframammary incision: an incision made in the breast crease between the breast and chest, through which breast implants are placed.
Capsular contracture: a breast augmentation complication in which otherwise normal scar tissue around a breast implant tightens and squeezes, causing a distorted breast appearance. Capsular contracture can be treated and reversed.
Subglandular placement: when the breast implants are placed behind the breast tissue but in front of the pectoral muscle.

Saline breast injections are not breast augmentation. Instead, this new fad involves injecting saline fluid into the breasts for a temporary, 24-hour enlargement effect. This use of injections is still awaiting FDA approval.
A recent article in the New York Times says many plastic surgeons have concerns about temporary breast enlargement with saline. First and foremost, the saline fluid may cause some stretching of the breast skin, which could potentially sag once the saline fluid is absorbed. Secondly, it provides only a temporary result.
Alternatively, consider some of the advantages of breast augmentation using saline or silicone breast implants:
- Long-lasting results. Breast implants may need to be replaced at some point, but they tend to last several years or possibly much longer.

Maybe. A recent poll shows many people identify the perfect or ideal breast based on nipple positioning. Specifically, when shown a photo of four breast types, most people polled identified the breast with a 45:50 upper pole to lower pole ratio. This ratio means 45 percent of the breast is above the nipple and 55 percent of the breast is below the nipple. Breast augmentation reshapes your breasts to the size and overall appearance that makes you look and feel great.
Researchers used photographs of women with various breast sizes. Each woman had four different pictures, with three of them modified in Photoshop to change the upper pole to lower pole ratios. The ratios were: 35:65, 45:55, 50:50, and 50:45. Then, 1,315 people were asked to view the images and rank the attractiveness of the breasts. The respondents included both genders, various ethnicities, and 53 plastic surgeons. Here are some of the results:
- Overall, the 45:55 ratio was rated as the most attractive across all the groups surveyed.
- The 45:55 ratio was favored by 90 percent of the male respondents.
- The 45:55 ratio was favored by 87 percent of women in their 30s.
- The 45:55 ratio was preferred by 94 percent of the plastic surgeons.
- The 50:50 ratio was the second most preferred, though not very closely.
The majority of people preferred the more natural 45:55 ratio to the fuller 50:50 ratio. This finding seems to indicate that men and women alike may prefer more natural breasts with gently sloping upper fullness to fuller breasts.
Ultimately, breast augmentation should achieve the breast size, shape and fullness that will make you feel the most confident about your body.

A breast lift is a widely popular procedure in this country. According to the American Society of Plastic Surgeons, breast lift procedures are up 70 percent since 2000. Women are typically very happy with the results of the mastopexy surgery. It lifts sagging, elongated breasts that have lost their shape and now droop downward.
A recent study says that many women may have to buy smaller bras after breast lift surgery, but not because their breasts are actually smaller. The lead researchers of the study concluded that, before having a breast lift, the women were buying the wrong bra size all along, probably because these larger bras feel more comfortable when your breasts have become stretched and elongated. After a breast lift, the breasts are returned to their true size, so you will again be comfortable and better supported by sizing down to your true bra size.
The study was published in the July issue of the Journal of Plastic and Reconstructive Surgery®. It included 20 women who underwent breast lift surgery. These women reported an average one cup decrease in bra size after the procedure, and none of them changed the brand of bra they were buying. They all reported being happy with their results.

I originally was trained on the use of a power assisted hand-piece to assist the surgeon aspirate fat with standard liposuction technique. Then during my training I also instituted the use of the Vaser System (Ultrasonic Assisted Liposuction). While the Vaser system was an effective adjunct to my liposuction practice, I have essentially stopped using ultrasonic energy and proceeded with a newer technique, SmartLipo. This is a laser assisted liposuction apparatus that not only melts the fat but also helps to tighten the skin. Most recently, I have once again, returned to using Power Assisted Liposuction developed by Microaire and I now combine this with the Smartlipo treatment of the fat. I truly find this combination to be most effective at contouring a patient's body shape and create the most impressive result yet!

A new study funded by Estee Lauder says that poor sleep quality is directly linked to your skin's ability to repair itself, which can affect the appearance and youthfulness of your face. If you have trouble sleeping - which many of us do - then you know how it affects your psychological wellbeing. This study looks more closely at how poor sleep quality can affect the way you look.
Consider some of the findings in the study:
Increased signs of aging: Lack of sleep resulted in more fine lines, uneven pigmentation and increased skin sagginess.
Ability to recover from sunburn: The skin of the sleep-deprived women seemed to have a harder time resolving inflammation caused by sun exposure. These women remained red for 72 hours after sun exposure, whereas the well-rested women had skin that recovered faster.
Retention of moisture: Staying hydrated is good for your body and good for the healthy appearance of your skin. The poor sleepers were less able to retain water than the good sleepers, so their skin was less effective in healing itself.
Work with your doctor to find out if there are ways to improve your sleep. Sometimes simple changes like increasing your physical activity or avoiding caffeine before bed can improve your sleep.

Breast lifts have been increasing in popularity over the past decade. According to new statistics from the American Society of Plastic Surgeons, breast lift procedures are growing at twice the rate of breast augmentations, although breast augmentation has a greater number of procedures performed each year and remains the most popular plastic surgery procedure among women.
Most of the patients who receive breast lifts are between 30 to 54 years old. If you are unhappy with loss of volume in your breasts that has led to a sagging, deflated breast appearance, then a lift may be right for you. This procedure can restore a more youthful shape and contour to your breasts, although it cannot increase size the way breast augmentation does.
In 2000, a total of 52,836 breast lift procedures were performed by ASPS member surgeons. In 2013, 90,006 breast lifts were performed. Those numbers represent an increase of 70 percent between 2000 and 2013. Comparatively, 290,224 breast augmentations were performed last year, an increase of 37 percent from the 212,500 breast augmentations performed in 2000.

It’s amazing how people generalize plastic surgeons. I’ve heard it said, “Well, you’re not a brain surgeon or a heart surgeon, it’s not like you’re saving lives….” But I would beg to differ. No, it’s not like someone is rushed into my OR and I have to do an emergency breast augmentation on them or they might die (although some patients do feel that way).
But every day, I get to meet people who have deliberated, searched their hearts and souls, and who at the end of the day have decided to come see me because on several levels they need a change.
I am in the business of changing lives, which, yes…. in some cases, saves lives.
People underestimate the power of personal appearance on how you feel on the inside, but I see it every day. Now, I personally feel that all people are beautiful. But I also see that they don’t feel beautiful. They don’t feel confident. They’ve tried self-help books. They’ve tried therapy and counseling. They’ve gone on the yoga retreat. They’ve changed their diet. They’ve beat themselves up and torn themselves down, and they come to my staff and sit in our practice, and it is here they start to feel a bit of hope.
Years ago, I had a 49 year old woman who was about 15 – 20 lbs overweight and did not have the body she once had. She was a busy professional (as most of you are!) and it was difficult to lose the weight that age and lifestyle had packed on. And in this case, even if she didn’t have the extra weight, she still had shape-related problems. (Now, please understand that for surgeons we don’t think that any particular shape is a problem, but if it’s a problem for you and it keeps you from achieving the look you want and feeling the way you desire – then it’s a ‘problem’. For example – if you are pear-shaped with a narrow ribcage and petite shoulder frame and yet you can’t stand your saddlebags and you’ve worked out for years and they’re still there? Or you have skinny, tiny legs and narrow hips but have broad back, shoulders and wide ribcage with no waist even at your thinnest? That’s a shape-related problem. Like any surgical procedure, nose surgery (also called rhinoplasty) requires you to take some downtime to recover. Every patient is unique, and your recovery will be unique, but some of the things rhinoplasty patients can typically expect while healing in the first couple of days include:
- Swelling and bruising around your nose and face
- Stuffiness in your nose
- Facial soreness
Prior to your procedure, you and our staff will discuss whether any medication is necessary for pain relief, and what you can do to alleviate swelling and pain, such as cold compresses and keeping your head elevated.
Generally, you can return to all of your normal activities in about seven to 10 days. During this time, a lot of the swelling will go down, although the majority of swelling will dissipate over two to four weeks. The final results of your rhinoplasty will mostly be visible within the first week or so, but very subtle changes in shape may take place over the next year as your tissues continue to heal and minor swelling continues to subside.

Smartlipo™ is a liposuction technology incorporating laser preparation of fat cells before they are removed. Some of the main advantages of this technique are:

Breast augmentation patients are various ages, from young women in their 20s to women age 40 and older. There is no right or wrong age for breast surgery. Some of the factors that can influence whether it's the right time in your life for breast augmentation are:

In a recent blog post, The American Society for Aesthetic Plastic Surgery (ASAPS) raised the issue that not all injectables, like botox, are safe or legal. Being that this is such a hot topic, we wanted to share Dr. Beck's thoughts on this very concerning issue - please don't take it lightly.
Are the fillers your physician or nurse using to augment your face legal or equal? As filler and injectable sales continually increase across the country, there are increasing illegitimate companies attempting to sell fillers and Botox to US trained physicians at reduced rates. For example, companies from Canada and Mexico email physicians and clinics around the country to entice them to buy the “same” product at reduced prices. This is especially attractive as the physicians and clinics compete for patients in this difficult economy, and charge lower prices for their services. As a result, the potential for harmful side effects and/or ineffective products has increased. A savings on the products may allow for clinics to sell their services at continued reduced rates to attract new patients. Bottom line is to ask your doctor or nurse where they get their products from. There are specific companies who sell these products directly to the end user, the doctors and nurses. If the clinic expresses they order them from another country, the probability of legitimacy issues may become problematic.

We loved watching the Victoria's Secret Fashion Show! Who doesn't love watching beautiful women flaunt their curves in gorgeous lingerie and fun costumes? One thing we could not keep our eyes off of, all the lovely cleavage! Since breasts are our specialty, we thought we would shed some light on which VS models have had a breast augmentation. May 5th – I was invited for the third year in a row to give an instructional course on Abdominoplasty at the American Society of Aesthetic Plastic Surgeons meeting. This interest was sparked by the paper Dr. Brink and I published in one of the most credible Plastic Surgery Journals back in May of 2009 in the Plastic and Reconstructive Surgery Journal. This year the meeting was held in the beautiful city of Vancouver, British Columbia!! Well over 2000 plastic surgeons attended this year’s event. The surgeons who attended were from all over the world and it’s a peer reviewed meeting. The instructional Course was titled “Abdominoplasty with Direct Deep Fat Resection.” The key points to this tummy tuck presentation were as follows:
- Use of liposuction at the time of abdominoplasty to the tummy tuck flap can be counter-productive.
- Use of liposuction to the tummy tuck flap prevents mobilization of the flap due to the inexactness of liposuction to stay within the deep fat.
- Less mobilization of the tummy tuck flap provides for a less than adequate result and correction of the contour of the abdomen especially above the belly button.
- Use of liposuction may present the patient with unevenness to the resulting tummy tuck.
- Therefore, why ever use liposuction at the time of Abdominoplasty?
- Rather, wide undermining of the flap with direct deep fat resection provides the best correction and enhances the end result, thus contour to the patient.
- This provides a smoother abdomen, tighter abdomen, and overall better patient satisfaction!!
This technique described in the published paper has been incorporated into every single one of our abdominoplasties for the last 10+ years. Our patient satisfaction has been excellent. Our corresponding OB/GYN’s of the patient’s we performed this procedure on have as well been very impressed. When considering having breast augmentation, there are many factors to consider. Choosing an implant size is a very personal decision that depends on what you are most comfortable with. The surgeon’s goal is to educate the patient so that they are able to make a well informed decision. The two options of implants are silicone gel and saline. The silicone used today is third generation cohesive gel which was FDA approved in November of 2006. After a fourteen year study it was found that there was no correlation between silicone implants and connective tissue disease. The saline or salt water filled implants have been used since the 1990’s. If you were to view photos of women with silicone or saline implants you probably would not be able to distinguish between the two.
The main difference between the two implants is in the “feel” of the implant. The silicone gel implants feel much more like “real” breast tissue then the saline implants. Going behind or in front of the muscle depends on breast shape and tissue coverage. If you have very thin skin and little breast tissue then going behind is usually a better choice. Typically silicone implants are the best choice for women who have very little breast tissue. If you have a “droopy” or sagging breast then in front of the muscle will create more fullness of the breast.
The implant incision site is also an important factor. You can place the implant through the transaxillary (armpit), inframammary (under breast) or periareola (through the areola). A very important factor to consider would be breast feeding. The periareolar incision should be avoided if you wish to breast feed in the future. This can cut into milk ducts and interfere with adequate milk production. Breast feeding is not affected by the implant being in front of or behind the muscle. Pregnancy is another important factor to consider when choosing size. It is hard to determine how pregnancy will affect your breasts. Usually the breasts will increase in size and there after when breast feeding. Most women wish they had gone larger and we usually encourage you to go with the largest volume you are comfortable with.
Size can also be hard to determine. With implants you can never be sure what cup size you will end up with. Everyone carries their implants differently. If you are very petite or narrow you will end up with a larger cup size with a relatively moderate implant volume. If you have a broader chest you can handle a larger volume implant without looking “top heavy”. A good tool for helping you determine what size implant to choose is the rice test. This allows you to play around with different volumes and get a feeling of how you will look with your clothes on. It is not exact but gives you a pretty good indication of size. Putting gel implants in your bra is a terrible representation of what you will look like.
Implants come in low, moderate and high profiles. Your surgeon will do a few measurements of your chest wall and this determines which profile is best for your body. The low profile has the least amount of projection and is widest in diameter. The moderate and high profile implants are narrower in diameter but have a higher projection. Things to consider as a prospective patient would be your physical activities. The larger the implant the more weighted it will be. It is very important to support your breast 24/7 and is recommended that you even sleep with a shelf bra at night.
It is important to view before and after photos with your plastic surgeon. Not everyone shares the same aesthetic goals. Also, it is important that you have realistic expectations. Breasts come in many shapes and sizes and what you consider to be “pretty” or “desirable” breasts may differ from one person to the next.

How do you keep your skin looking good? By taking the following proactive steps.
1. Always use sunblock. Sun damages skin. UV-B rays penetrate the top layers of skin and are most responsible for sunburns. UV-A rays penetrate to the deeper layers of the skin. Use a sunblock of at least 30 SPF with zinc oxide to make sure it protects against UV-A and UV-B. Apply every two hours or after getting out of the water. Sunblock washes off and breaks down after application. 2. Avoid laying out in the sun or tanning beds. Spray tans or airbrush tan available at retail outlets provide the glow you’re looking for – without exposing your skin to harmful sunlight to tanning lamps. The World Health Organization’s International Agency for Research on Cancer (IARC) recently put UV tanning beds into its highest cancer risk category -- "carcinogenic to humans." Tanning salons typically use lamps that emit both UV-A and UV-B radiation. 3. Apply eye cream regularly. Moisturizing the area around your eyes can help reduce wrinkles, but it pays to do your research. Eye cream, which comes in day and night formulas, is a cosmetic and therefore not approved by the FDA. When buying eye cream, as well as other anti-aging products, look for ingredients such as retinoids (retinol, retinyl palmitate, retinyl propionate, and others), peptides, vitamin C, niacinamide, caffeine, grape seed extract, marine extract, and other antioxidants. 4. Use Topical Vitamin C. Vitamin C lotion can help reduce the amount of free radical damage that occurs from sun exposure.
